As the home health industry becomes more consumer-driven and addresses changing regulations such as PDGM, providers need proven solutions that engage patients to improve experiences and outcomes. However, there are few studies that demonstrate the impact of patient engagement on these key measures.
In this research article, we explore the impact of patient engagement throughout the care episode and post-episode follow-up. As the study and evidence suggests, keeping patients engaged demonstrates fewer avoidable re-hospitalizations and improved HHCAHPS scores.
